Medical Clearance & Consent Policy

In accordance with Kentucky State law, all new clients — and established clients requesting a new type of treatment — must receive pre-treatment clearance from our medical director prior to receiving services. All clients are also required to complete and sign treatment consent forms before treatment. Medical clearance and consent documentation must be renewed annually.
